The problem

An AI-drafted proposal goes to a client with the wrong contract term. Four people touched it. In the review meeting: the analyst assumed the manager would check commercials; the manager assumed the analyst had verified them because they wrote the prompt; and everyone assumed the AI had been given the right terms.

Verify before you use it

• Is every checklist item answerable in 30 seconds? If an item requires ten minutes of investigation it will be skipped under deadline.
• Is the Approver a single named person? 'The team approves' means nobody does.
• Do the stop conditions describe rejection rather than editing? Editing a fundamentally wrong output usually preserves the wrongness in a new form.
• Has the 'do not spend time on' list actually been communicated? Reviewers default to proofreading unless explicitly redirected.
