- Is every checklist item answerable in 30 seconds? If an item requires ten minutes of investigation it will be skipped under deadline.
- Is the Approver a single named person? 'The team approves' means nobody does.
- Do the stop conditions describe rejection rather than editing? Editing a fundamentally wrong output usually preserves the wrongness in a new form.
- Has the 'do not spend time on' list actually been communicated? Reviewers default to proofreading unless explicitly redirected.
